MedStar St. Mary's Hospital partners with MedStar National Rehabilitation Hospital in Washington, D.C. The Transitions Neuro Day Treatment Program at MedStar National Rehabilitation Hospital is designed for people who are medically stable and able to live at home, but whose daily lives can be improved through organized, intensive outpatient therapy within a daily routine. Our goal is to help the individual gain independence, whether it be at home, at work or in the community.

This dynamic program stresses both functional and practical individual and group therapy sessions that are tailored to the individual. These sessions include physical, occupational, and speech therapy, as well as neuropsychology and social work services. A patient typically attends the program four to five days per week, and participates in a half-day of treatments that are coordinated into morning and afternoon tracks; individuals may progress from one track to another during their course of treatment within the program.

Each daily session can last up to six hours. The average length of time in the program is 8.5 weeks. Because we use an interdisciplinary approach to care, each patient's progress is evaluated regularly by our team at biweekly conferences. Most services are delivered in the outpatient department, Independence Square, the therapeutic garden, and as needed, the pool and/or the Seating and Mobility Clinic. In addition, you can expect the following:

  • Small-group outings (pictured above) which promote mobility and independence in the community
  • Development of language skills by our team of speech-language pathologists
  • Involvement of family members and caregivers through specialized training and education sessions
  • Interpreter services are not provided for those with limited English proficiency, but are available for patients who are hearing impaired
  • Typically, there are approximately 34 visits of speech therapy, 26 visits of occupational therapy, and 22 visits of physical therapy, and 8 visits of neuropsychology (2016 data)

Admission Criteria 

Referrals may be made by anyone associated with the patient, including physicians, other treatment providers, or family/support persons; however, physician’s orders for PT, OT, and SLP services are needed before individuals can be evaluated and treated.

Transition and Exit Criteria

Goals for each individual patient are established upon admission to the program, and are routinely updated during the patient’s episode of care. Patients may be transitioned to various groups and therapies as needed throughout their treatment in the program, based upon individual goals and level of functioning. Patients are discharged from the program when they no longer have group or outing based goals, no longer have goals for OT or SLP, or are no longer making progress towards their goals. In many cases following discharge from the day program, patients may benefit from additional treatment in a traditional outpatient therapy setting. Referrals to outpatient services or other services in the community are provided upon discharge from the Day Program.

Cost of the Program

The program works with several payer sources, including several insurances and worker’s compensation.
