What is a Primary Care Physician?
Primary care physicians (PCPs) are specially trained to serve as a patient's main doctor over the course of many years. Although your health plan may not require you to have a PCP, having one offers several advantages. Your PCP gets to know you, your family history, and your medical history—knowing this about your overall health, he or she can offer the best treatment advice. And, because your PCP keeps track of your screenings, he or she can remind you to schedule them, which can help detect problems earlier. Learn more about the importance of Primary Care.
Whether you are pregnant and need care through delivery and care for your child, an older adult needing specific treatment for your advanced years, or a child or adult with intellectual and developmental disabilities, our specialists can help. We take care of our patients in the hospital, the doctor's office, the home, and the nursing home—available for whatever your care may need.

Our program provides clinical care coordination performed by registered nurses, who will visit your home and make a Care Plan that meets your needs. He or she will go over your medical history with you and record all your medications, learn about any conditions or diseases you may have, and review your diet with you. This will help you stay well and at home in your community. If needed, a clinical care coordinator will accompany you to healthcare appointments.
